ClinicForms is a web application designed to make life easier for massage therapists, acupuncturists, chiropractors and other hands‑on practitioners.
It replaces paper forms with a fast, secure, and easy digital process that works on any phone, tablet, or computer.

Most therapists spend too much time printing, storing, and organising paper intake forms. ClinicForms removes that stress. Clients complete their forms online before the appointment, and you receive everything directly in your email inbox.

No accounts. No setup. No data stored on our servers. Just simple, compliant record‑keeping.

1. Client enters their information

Clients fill in their personal details and health information on their phone or computer.

2. Client reads the consent form

All consent information is clearly displayed in simple language.

3. Client signs with their finger

A digital signature is captured directly on the screen.

4. You receive the completed form

A PDF copy is sent to your email address (and to the client if they choose). ClinicForms v 1.0.0 does not store any client data on the cloud.

ClinicForms temporarily stores completed forms on the device’s flash memory for 7 days, after which they are automatically deleted.

5. Archiving completed client forms

The user receives an email with the PDF attached. These forms can be stored on the therapist’s device (laptop, phone, or tablet), or printed as a hard copy for reference.

We are now developing:

• Chiropractic Patient Intake Form

• Additional forms based on therapist and organisation feedback

ClinicForms was created by a practising therapist who understands the daily challenges of record‑keeping. It is built to support:

• Legal and professional requirements

• Simple, secure documentation

• A smoother experience for both therapist and client

There will always be a free tier available for individual therapists.

ClinicForms is developed by CareTrace Ltd in cooperation with the Software Development Team at Edinburgh College. We continue to work with therapists, training organisations, and professional bodies to improve the system.
